Grant Snapshot: Strengthening connections between frontline health, outreach, and community service providers supporting people with complex care needs across the Saanich Peninsula. Funding will support a series of collaborative gatherings that bring together Island Health, First Nations health services, outreach organizations, and grassroots volunteers to improve communication, referrals, and coordinated responses for individuals experiencing mental health, addictions, housing, and health challenges.
